Siya Kolisi will make his first appearance for the Stormers in more than five years when they face Edinburgh in the United Rugby Championship at the Hive Stadium on Friday, 2 October.
Kolisi, who began his professional career with the Cape Town side, will win his 119th Stormers cap when the match kicks off at 20:45 South African time.
He starts at openside flank, with Louw Nel moving to blindside and Riley Norton coming into the second row. Ntuthuko Mchunu is the only other change to the forward pack, as he comes into the front row.
The Stormers have also made two changes among the backs, with Stefan Ungerer and Yaqeen Ahmed selected at halfback.
Wandile Mlaba is the only new addition to the replacements bench, where he provides cover among the loose forwards.
Director of Rugby John Dobson said the Stormers were determined to complete their tour with a victory before returning to Cape Town for four successive home matches.
“Last week’s performance against Connacht was encouraging in many ways, but we would like to see a more ruthless edge to our game, which we have worked on this week,” he said.
“We know that Edinburgh is a tough place to play and we have some experience coming in, with Siya, Ntuthuko and Stefan included along with Yaqeen Ahmed and Wandile Mlaba, who are both really exciting young players.
“It will take a big effort from all 23 players to get the result before we go back to Cape Town for what will be a crucial period in the context of our season.”
Ruhan Nel will captain the team from outside centre.
The Stormers side is:
